Carboxytherapy refers to the cutaneous and subcutaneous administration of carbon dioxide gas [CO2] for therapeutic purposes. Carboxytherapy originated at the Royal Spas of France in 1932 with the treatment of patients afflicted by peripheral arterial occlusive disease. In South America and Europe, carbon dioxide therapy has been applied to the treatment of stretch marks, cellulite, and hypertrophic scars with impressive results. Studies have demonstrated that carboxytherapy improves skin elasticity, improves circulation, encourages collagen repair, improves the appearance of fine lines and wrinkles, and destroys localized fatty deposits. Carboxytherapy – The Mechanics
Carbon dioxide is present in our bodies at all times. We breathe in oxygen, and we exhale carbon dioxide. Plants take up the carbon dioxide, and in turn give us the oxygen that we need. Carbon Dioxide also happens to be the signal for poor blood circulation in the body. All cells in the body, regardless of their job (heart cells, brain cells, skin cells) release as their waste product carbon dioxide. Carbon dioxide is the “cost of doing business” of any cell in our bodies. So, we breathe in oxygen to our lungs, the red blood cells pick up the oxygen in our lungs and carry it to our tissues until they encounter an area that has been working hard and has an excess of carbon dioxide. When the blood cells are exposed to high concentrations of carbon dioxide, they flip their conformation, release the oxygen molecules, and pick up the carbon dioxide so that we can exhale it from our lungs. In a sense, by injecting small amounts of carbon dioxide gas just below the surface of the skin, we are tricking it into increasing the blood circulation to that area. Dark under-eye circles, cellulite, and stretch marks have all been shown to have some root cause in poor blood circulation.
Carboxytherapy for Stretch Marks
Stretchmarks are characterized by wide linear bands of discolored or wrinkled skin that occurs in areas of collagen damage due to stretching. Striae are usually found on the abdomen, buttocks, thighs, and breasts. Women develop striae more commonly than men, and striae are seen in 90% of pregnant women due to a combination of hormonal factors along with increased stress on the collagen and elastin in the skin. Some studies indicated that striae are a form of scar tissue that forms in response to collagen rupture and elastic fiber changes. Two types of striae are described, newly formed, reddish colored and older, white striae. The treatment of old striae has been challenging and various modalities have been studied. These include topical retinoids (Retin A), microdermabrasion, intense pulsed light (IPL) and fractionated laser skin resurfacing. Although striae are thought to be a result of collagen rupture and breakdown during stretching of the skin, until recently, few treatments actually targeted the root cause of the striae formation, the damaged collagen in the dermal layer. Furthermore, laser and light modalities are not safe for individuals with darker skin tones. Carboxytherapy is the only treatment available for both old and new stretchmarks that visibly repairs the broken collagen and is safe for all skintypes. Carboxytherapy causes the formation of new collagen and subsequently thickens the skin to improve the appearance of the stretchmarks by rebuilding the collagen matrix.
Carboxytherapy for Cellulite
Cellulite refers to the lumpy fat bulges on the thighs and buttocks of over 95% of the normal female population. There are many reasons why most women are predisposed to cellulite whereas the condition is much rarer in men. To begin with, the underlying structure of the skin is different in males verses females. Male skin tends to be thicker and the fibrous septae has strong cross-linking of the connective tissue. Females have thinner skin and no cross-linking of the underlying septae. Women have more fat layers than men, and the subcutaneous fat layer in women is regulated by hormones and does not respond to diet and exercise. The female hormone estrogen causes these fat cells to store fat, whereas the male hormone testosterone stimulates the fat cells to break down fat. So, women are genetically superior at storing energy in the form of fat to provide energy during pregnancy. Women also have a higher percentage of body fat in the areas of the thighs, hips, and abdomen, and these fat cells are resistant to diet and exercise. Directly beneath the fat layer there is a layer of connective tissue comprised of collagen called the “fibrous septae.” When the fibrous septae becomes damaged, the subcutaneous fat cells are pushed through the damaged regions and are squeezed into small bulges that give the overlying skin the “puckered” or “dimpled” appearance that we call cellulite.
Women have tried numerous therapies to eliminate their cellulite including various creams, massage, etc to no avail because none of these therapies correct the underlying physiological problems of poor circulation and damaged collagen septae. Two therapies that have shown promise in eliminating this difficult problem are the Thermage CL cellulite tip and carboxytherapy. Carboxytherapy was originally used for aesthetic purposes by the Brazilians to sculpt residual post-liposuction fatty deposits above the knees and histological studies showed that the fat cells were ruptured by the CO2 gas while leaving the remaining skin structures and nerves unharmed. Collagen remodeling was also shown to occur, as well as thickening and smoothing of the overlying skin.

Carboxytherapy can dramatically improve the appearance of dark under-eye circles. Although sometimes caused by darkened pigment, or a hollow depression below the lower eyelids [tear trough deformity], the majority of dark under-eye circles are caused by poor circulation beneath the lower eyelids. WHAT KIND OF TECHNOLOGY DOES THE ULTRAFORMER USE?
The ULTRAFORMER uses MF HIFU (Multi Functional High Intensity Focused Ultrasound). MF HIFU affects various layers of the superficial dermis, deep dermis, fat layers including the SMAS (Superficial Muscular Aponeurotic System). The SMAS are the deeper layers which in a surgical face lift a surgeon would lift under the knife. Botox Injection
Botox injection is commonly performed at plastic surgicentre in Doha, Qatar.
Botulinum toxin (Botox) is a toxin produced by the bacterium Clostridium botulinum. It causes muscle paralysis.
Botox was originally introduced for the safe and effective treatment of muscle spasms. In patients treated for hemifacial spasm it was noted that facial wrinkling decreased over the treated muscle. This experience lead to the development of Botox for the treatment of facial wrinkles. People often dislike their central frown line, which is caused by concentrating, squinting, or frowning. Botox injections can successfully reduce its prominence.
Botox can also be used to lessen crow’s feet and other expression lines.
Tiny quantities of the toxin are injected directly into the muscles of facial expression. It takes three small injections between the eyebrows to treat the frown line. The treated muscles weaken over the following week or so. Most people do not notice anything. They simply become aware that they are no longer able to contract the frown muscles. They can still lift their eyebrows normally and blink without problems. The injection is almost painless.
It is important to remain upright for four hours after the injection. The treated areas should not be touched.
The effect lasts between four and six months. Treatment can be repeated as required.
Studies have also suggested that Botox is effective in relieving migraine headaches, excessive underarm sweating and muscle spasms in the neck and eyes.
Some people have a slight headache after treatment for several hours. The most common significant complication, which is rare, is “ptosis”. This is a drooping of the eyelid caused by the Botox tracking into the eyelid muscle. It generally lasts just a few days, but more prolonged weakness is possible.
Botox injections cannot be used in pregnancy or when breast feeding.

Immediately following treatment, you may notice some minor discomfort, or stinging in the injected area. Occasionally some bruising or swelling will occur, but it is usually minor. No bandaging is needed and you are free to eat, drink, and wear makeup with sunblock protection shortly thereafter. There may be some temporary swelling and redness in the treated area which should dissipate within a few days. If these symptoms persist, contact your surgeon.

What can you expect during an office visit for platelet rich plasma? First, we draw a small amount (about 10cc) of blood. The separation process, requiring a centrifuge, takes approximately 9 minutes. Once the PRP serum is ready, it’s placed in a syringe; and ready for injection. The serum is thin enough to use a very narrow needle — the same, small-caliber, needle size normally used to administer Botox. Unlike Botox and fillers, please be aware there is a recovery period involving swelling; and in some cases, bruising. PRP does cause an inflammatory response; so this is expected. Once these effects subside, patients will notice that lines and wrinkles have softened; and areas of volume loss, such as nasolabial (the area between your nose and upper lip) folds, look fuller.

Chemical Peel
If you’re considering chemical peel …
Chemical peel is especially useful for the fine wrinkles on cheeks, forehead, and around the eyes, and the vertical wrinkles around the mouth.
Chemical peel uses a chemical solution to improve and smooth the texture of the facial skin by removing its damaged outer layers. It is helpful for those individuals with facial blemishes, wrinkles and uneven skin pigmentation. Phenol, trichloroacetic acid (ITCA) and alphahydroxy acids (AHAs) are used for this purpose. The precise formula used may be adjusted to meet each patient’s needs. Although chemical peel may be performed in conjunction with a facelift, it is not a substitute for such surgery, nor will it prevent or slow the aging process. The following information provides basic information about certain types of chemical peel treatments and the results you might expect. It won’t answer all your questions, since a lot depends on your individual circumstances. Once you and your plastic surgeon have decided on a specific peel program, be sure to ask about any details that you do not understand.

Deciding if chemical peel is right for you
Chemical peel is most commonly performed for cosmetic reasons – to enhance your appearance and your self confidence. The chemical solution can be applied to the entire face, or to a specific area – for example, around the mouth – sometimes in conjunction with a facelift.
Alphahydroxy acids (AHAs), such as glycolic, lactic, or fruit acids are the mildest of the peel formulas and produce light peels. These types of peels can provide smoother, brighter-looking skin for people who can’t spare the time to recover from a phenol or TCA peel. AHA peels may be used to treat fine wrinkling, areas of dryness, uneven pigmentation and acne. Various concentrations of an AHA may be applied weekly or at longer intervals to obtain the best result. Your doctor will make this decision during your consultation and as the treatment proceeds. An alphahydroxy acid, such as glycolic acid, can also be mixed with a facial wash or cream in lesser concentrations as part of a daily skin-care regimen to improve the skin’s texture.
Trichloroacetic acid (TCA) can be used in many concentrations, but it is most commonly used for medium-depth peeling. Fine surface wrinkles, superficial blemishes and pigment problems are commonly treated with TCA. The results of TCA peel are usually less dramatic than and not as long-lasting as those of a phenol peel. In fact, more than one TCA peel may be needed to achieve the desired result. The recovery from a TCA peel is usually shorter than with a phenol peel.
Phenol is the strongest of the chemical solutions and produces a deep peel. It is used mainly to treat patients with coarse facial wrinkles, areas of blotchy or damaged skin caused by sun exposure, or pre-cancerous growths. Since phenol sometimes lightens the treated areas, your skin pigmentation may be a determining factor as to whether or not this is an appropriate treatment for you. Phenol is primarily used on the face; scarring may result if it’s applied to the neck or other body areas.
All chemical peels carry some uncertainty and risk
At the end of a phenol peel, a thick layer of petroleum jelly may be applied to the treated area.
Chemical peel is normally a safe procedure when it is performed by a qualified, experienced plastic surgeon. However, some unpredictability and risks such as infection and scarring, while infrequent, are possible.
AHA peels may cause stinging, redness, irritation and crusting. However, as the skin adjusts to the treatment regimen, these problems will subside.
With a TCA peel, your healed skin will be able to produce pigment as always; the peel will not bleach the skin. However, TCA-peel patients are advised to avoid sun exposure for several months after treatment to protect the newly formed layers of skin. Even though TCA is milder than phenol, it may also produce some unintended color changes in the skin.
With a phenol peel, the new skin frequently loses its ability to make pigment (that is, tan). This means that not only will the skin be lighter in color, but you’ll always have to protect it from the sun. Phenol may pose a special risk for patients with a history of heart disease. It’s important that you make your surgeon aware of any heart problems when your medical history is taken.
It is also possible that phenol will cause some undesired cosmetic results, such as uneven pigment changes. Certain modified phenol peels are gentler and may be preferred in some circumstances.

Planning for a chemical peel
A protective crust may be allowed to form over the new skin. When it’s removed, the skin will be bright pink.
In some states, no medical degree is required to perform a chemical peel – even the strongest phenol peels. Many states have laws that permit non-physicians to administer certain peel solutions, but regulate the strengths which they are permitted to apply. You should be warned that phenol and TCA peels have been offered by inadequately trained practitioners claiming “miracle techniques” to rejuvenate the skin.
It is very important that you find a physician who has adequate training and experience in skin resurfacing. Your plastic surgeon may offer you a choice of peel techniques or suggest a combination of peels to obtain the best result for you.
During your initial consultation, it is important that you discuss your expectations with your plastic surgeon. Don’t hesitate to ask any questions or express any concerns that you may have. Expect your plastic surgeon to explain the planned procedure in detail, including its risks and benefits, the recovery period and the costs. If you have a history of herpes, you should inform your physician prior to the procedure. Remember, chemical peel treatments are usually not covered by medical insurance unless they are performed for medically related problems.
Preparing for your chemical peel
Your plastic surgeon will instruct you on how to prepare for your peel treatment.
Sometimes Retin-A – a prescription medication derived from Vitamin A – is used to pre-treat the skin. This thins out the skin’s surface layer, allowing the TCA solution to penetrate more deeply and evenly. If your skin won’t tolerate Retin-A pre-treatment, an AHA cream may be used instead. Hydroquinone, a bleaching agent, is sometimes used in conjunction with Retin-A or AHA pre treatment, especially if you have blotchy skin areas or pigmentation problems. You may have to spend a month or more in the pre-treatment phase before the doctor will schedule your actual peel.
You will need to arrange for someone to drive you home and help you out for a day or two if you are having a phenol or deeper TCA peel. You probably won’t need any extra assistance if you’re having an AHA peel or superficial TCA peel.
Where your peel will be performed
Most chemical peels may be safely performed in a plastic surgeon’s office, office-based surgical facility or outpatient surgical center. Your plastic surgeon may want you to stay overnight in a facility or hospital if other cosmetic procedures are performed simultaneously.
Types of anesthesia
Anesthesia isn’t required for phenol or TCA peels because the chemical solution acts as an anesthetic. However, sedation may be used before and during the procedure to relax you and keep you comfortable.
No anesthesia is needed for AHA peels since they cause only a slight stinging sensation during application.
The peel
After healing, the skin is lighter in color, tighter, smoother, and younger looking.
AHA peels/treatments: Your doctor will apply the AHA solution to your cleansed facial skin, a process that usually takes no more than 10 minutes. No “after-peel” ointment or covering is required. Depending on the strength of the peel, periodic treatments may be necessary until the desired effects are achieved.
For some patients, the application of an AHA-based face wash or cream once or twice a day at home will be sufficient to accomplish the desired goal. Your plastic surgeon may add Retin-A or a bleaching agent to your at home treatment schedule. After several weeks of at-home use, your doctor will examine your skin to determine if your regimen needs adjustment.
Phenol and TCA peels: Typically, the skin is first thoroughly cleansed. Then, the surgeon will carefully apply the phenol or TCA solution. You may feel a stinging sensation as the peel solution is applied, but this feeling will quickly pass.
A full-face TCA peel usually takes no more than 15 minutes. Two or more TCA peels may be needed to obtain the desired result, and those may be spaced out over several months. Mild TCA peels may be repeated as often as every month.
If phenol solution has been used, your plastic surgeon may coat the treated area with petroleum jelly or a waterproof adhesive tape. With lighter peels, no covering is necessary.
A full-face phenol peel generally takes one or two hours to perform, while a phenol peel to a smaller facial region (perhaps the skin above the upper lip) may take only 10 or 15 minutes. A single treatment usually suffices.
After your treatment
After an AHA peel, it is common to experience some temporary flaking or scaling, redness and dryness of the skin. However, these conditions will disappear as the skin adjusts to treatment.
After a phenol or TCA peel, your doctor may prescribe a mild pain medication to relieve any tingling or throbbing you may feel. If tape was used to cover your face, it will be removed after a day or two. A crust or scab will form on the treated area. To help your face heal properly, it is essential that you follow your doctor’s specific post-operative instructions.
A TCA peel may also cause significant swelling, depending on the strength of the peel used.
If you’ve had a phenol peel, your face may become quite swollen. Your eyes may even be swollen shut temporarily. You will need someone to help care for you for a day or two. You may also be limited to a liquid diet and advised not to talk very much during the first few days of recovery.
Getting back to normal
With an AHA peel, the temporary redness, flaking, and dryness that you experience will not prevent you from working or engaging in your normal activities. A fresher and improved skin texture will result with continued AHA treatments. Remember, protecting your skin from the sun is also important following these mild acid peels. Ask your doctor to recommend a sunblock with adequate UVA and UVB protection and use it every day.
With a TCA peel, the moderate discomfort and mild swelling you may experience will subside within the first week. In about a week to ten days, your new skin will be apparent and you should be healed sufficiently to return to your normal activities. It is best to avoid sun exposure unless you are adequately protected
With a phenol peel, new skin will begin to form in about seven to ten days. Your face will be very red at first, gradually fading to a pinkish color over the following weeks to months. During this time, it is especially important that you use a sunblock or blotchy, irregular skin coloring may result.
About two weeks after treatment, you may return to work and resume some of your normal activities. Your skin will be healed enough for you to wear makeup. (For makeup tips, ask your plastic surgeon for the ASPRS brochure on camouflage cosmetics.)
Your new look
Improvements from AHA peels may be very subtle at first. You may detect a healthier glow to your skin. With continued treatments, you will notice a general improvement in the texture of your skin.
The results of a TCA peel are usually not as long-lasting as those of phenol peel. However, your skin will be noticeably smoother and fresher-looking.
If you’re planning a phenol peel, you can expect dramatic improvement in the surface of your skin – fewer fine wrinkles, fewer blemishes and more even-toned skin. Your results will be long-lasting, although not immune to the effects of aging and sun exposure.

Acne treatment by blue light
Acne is one of the most common skin conditions in Qatar, affecting over 85% of adolescents by the age of 24 years and up to 50% of adults over 25 years of age. Acne is a widespread and upsetting condition. It can produce lifelong physical and emotional scarring.
Acne is NOT caused by bad diet or greasy foods. In fact, acne develops when the sebaceous glands in a person’s skin produces excess oil? (sebum). This oil combines with cells that line the gland walls and clogs the skin’s pores. Normal skin bacteria colonise these pores and the body’s response causes the inflammation or reddening of the skin that we associate with acne.

Vitilligo
Vitiligo is a disorder in which white patches of skin appear on different parts of the body. This happens because the cells that make pigment (color) in the skin are destroyed. These cells are called melanocytes. Vitiligo can also affect the mucous membranes (such as the tissue inside the mouth and nose) and the eye.
What Causes Vitiligo?
The cause is not known. Vitiligo may be an autoimmune disease. These diseases happen when your immune system mistakenly attacks some part of your own body. In vitiligo, the immune system may destroy the melanocytes in the skin. It is also possible that one or more genes may make a person more likely to get the disorder.
Some researchers think that the melanocytes destroy themselves. Others think that a single event such as sunburn or emotional distress can cause vitiligo. But these events have not been proven to cause vitiligo.
Who Is Affected by Vitiligo?
Many people develop it in their twenties, but it can occur at any age. The disorder affects all races and both sexes equally, however, it is more noticeable in people with dark skin.
People with certain autoimmune diseases (such as hyperthyroidism) are more likely to get vitiligo than people who don’t have any autoimmune diseases. Scientists do not know why vitiligo is connected with these diseases. However, most people with vitiligo have no other autoimmune disease.
Vitiligo may also run in families. Children whose parents have the disorder are more likely to develop vitiligo. However, most children will not get vitiligo even if a parent has it.
What Are the Symptoms of Vitiligo?
White patches on the skin are the main sign of vitiligo. These patches are more common in areas where the skin is exposed to the sun. The patches may be on the hands, feet, arms, face, and lips. Other common areas for white patches are:
The armpits and groin (where the leg meets the body)
Around the mouth
Eyes
Nostrils
Navel
Genitals
Rectal areas.
People with vitiligo often have hair that turns gray early. Those with dark skin may notice a loss of color inside their mouths.
Will the White Patches Spread?
There is no way to tell if vitiligo will spread. For some people, the white patches do not spread. But often the white patches will spread to other areas of the body. For some people, vitiligo spreads slowly, over many years. For other people, spreading occurs quickly. Some people have reported more white patches after physical or emotional stress.
How Is Vitiligo Diagnosed?
The doctor will use a family and medical history, physical exam, and tests to diagnose vitiligo. The doctor may ask questions such as:
Do you have family members with vitiligo?
Do you or family members have any autoimmune diseases?
Did you have a rash, sunburn, or other skin problem before the white patches appeared?
Did you have some type of stress or physical illness?

The doctor will do a physical exam to rule out other medical problems.

How Is Vitiligo Treated?
Treatment may help make the skin look more even. The choice of treatment depends on:
The number of white patches
How widespread the patches are
The treatment the person prefers to use.
Some treatments are not right for everyone. Many treatments can have unwanted side effects. Treatments can take a long time, and sometimes they don’t work.
Current treatment options for vitiligo include medical, surgical, and other treatments. Most treatments are aimed at restoring color to the white patches of skin.
Medical treatments include:
Medicines (such as creams) that you put on the skin
Medicines that you take by mouth
A treatment that uses medicine plus ultraviolet A (UVA) light (PUVA)
Removing the color from other areas so they match the white patches.
Surgical treatments include:
Skin grafts from a person’s own tissues. The doctor takes skin from one area of a patient’s body and attaches it to another area. This is sometimes used for people with small patches of vitiligo.
Tattooing small areas of skin.

Psorisis
Psoriasis is a skin condition that causes red, flaky, crusty patches of skin covered with silvery scales.
These patches normally appear on your elbows, knees, scalp and lower back, but can appear anywhere on your body. Most people are only affected with small patches. In some cases, the patches can be itchy or sore.
Psoriasis affects around 2% of people. It can start at any age, but most often develops in adults under 35 years old. The condition affects men and women equally.
The severity of psoriasis varies greatly from person to person. For some people it’s just a minor irritation, but for others it can have a major impact on their quality of life.
Psoriasis is a long-lasting (chronic) disease that usually involves periods when you have no symptoms or mild symptoms, followed by periods when symptoms are more severe.

Why it happens
People with psoriasis have an increased production of skin cells.
Skin cells are normally made and replaced every three to four weeks, but in psoriasis this process only lasts about three to seven days. The resulting build-up of skin cells is what creates the patches associated with psoriasis.
Although the process isn’t fully understood, it’s thought to be related to a problem with the immune system. The immune system is your body’s defence against disease and infection, but for people with psoriasis, it attacks healthy skin cells by mistake.
Psoriasis can run in families, although the exact role that genetics plays in causing psoriasis is unclear.
Many people’s psoriasis symptoms start or become worse because of a certain event, known as a “trigger”. Possible triggers of psoriasis include an injury to your skin, throat infections and using certain medicines.
The condition isn’t contagious, so it can’t be spread from person to person.
How psoriasis is diagnosed
Your GP can often diagnose psoriasis based on the appearance of your skin.
In rare cases, a small sample of skin, called a biopsy, will be sent to the laboratory for examination under a microscope. This determines the exact type of psoriasis and rules out other skin disorders, such as seborrhoeic dermatitis, lichen planus, lichen simplex and pityriasis rosea.
You may be referred to a dermatologist (a specialist in diagnosing and treating skin conditions) if your doctor is uncertain about your diagnosis, or if your condition is severe.
If your doctor suspects you have psoriatic arthritis, which is sometimes a complication of psoriasis, you may be referred to a rheumatologist (a doctor who specialises in arthritis). You may have blood tests to rule out other conditions, such as rheumatoid arthritis, and X-rays of the affected joints may be taken.
Treating psoriasis
There’s no cure for psoriasis, but a range of treatments can improve symptoms and the appearance of skin patches.
In most cases, the first treatment used will be a topical treatment, such as vitamin D analogues or topical corticosteroids. Topical treatments are creams and ointments applied to the skin.
If these aren’t effective, or your condition is more severe, a treatment called phototherapy may be used. Phototherapy involves exposing your skin to certain types of ultraviolet light.
In severe cases, where the above treatments are ineffective, systemic treatments may be used. These are oral or injected medicines that work throughout the whole body.
